Participant Acknowledgment and Assumption of Risk
I acknowledge that my participation in activities, classes, or events organized by ZonAtletica involves inherent risks, including but not limited to physical exertion, injury, illness, or unforeseen accidents. I assume all risks, known and unknown, associated with physical activity or use of ZonAtletica's facilities, equipment, or programming—whether in-person or virtual.
Release and Waiver of Liability
In consideration of being permitted to participate in any ZonAtletica activity, I waive, release, and discharge ZonAtletica, its owners, instructors, employees, contractors, and affiliates from any and all liability, claims, demands, or causes of action related to any loss, damage, injury, or illness incurred during participation. This waiver extends to all claims of every kind and nature, to the fullest extent permitted by law.
Medical Clearance and Emergency Authorization
I confirm that I am physically able to participate and have no medical condition preventing me from doing so safely. I grant permission for emergency medical treatment if needed.
Photo/Video Release (Optional)
By signing this form, I grant ZonAtletica the right to use photos or video of me for marketing purposes unless I opt out by notifying staff in writing.
Agreement
I affirm that I have read, understood, and voluntarily agree to all terms. I certify that I am 18 years of age or older. If under 18, a parent or guardian must complete this form.
